Please read the "Dear Prospective Adoptive Parents"
letter carefully, print it out, and save it for your files. Then print out the
Application, RIL and
Fee Agreement forms,
fill them out and sign them. Please complete section II of the RIL form, and note that you must submit a separate form for each person over the age of 18 living in the home. Send these three documents along with your check to:

Electronically
You can fill out the Application online, send it to our office electronically, and pay for the report by credit card online, utilizing PayPal.
If you choose to pay electronically, with a credit card or debit card:
- For a STANDARD homestudy the fee is $1,486
*includes a $36 electronic filing fee
- For an EXPEDITED homestudy the fee is $1,793
*includes a $43 electronic filing fee
If you are ordering other types of reports, simply add 2.5% to your fee for electronic payments.
Please note that we must receive your full payment in order to open your file and begin working with you.
Terms and Conditions
Preplacement and Post Placement Assessments require at least one visit to the home. A surcharge may be applied to cover travel time for the social worker. Actual mileage expenses are to be paid directly to the social worker at the rate of $.60 cents per mile and $50 per hour (round trip) for the social worker's travel time. All travel fees must be paid in full before the final Preplacement Assessment is released.
The undersigned agree that when payment is received, Nathanson Adoption Services will complete the appropriate assessment. The fee agreement is not based on the outcome of the report nor the adoption proceeding. Once the process has begun fees are not refundable.
We the undersigned authorize Nathanson Adoption Services to release information relevant to our Assessment to our child-placing agency or attorney. We understand this consent can be rescinded at any time and shall remain valid until the completion of the adoption.
